- I-20 Request Form - Need a new form I-20? You may, if you are changing your major or have lost or damaged your original I-20.
- Leave of Absence/Early Withdrawal (immigration status only) - Are you thinking about leaving the country and need to let us know? Do you plan on returning at a later date, but are not eligible for a vacation quarter?
- Letter Request - Planning to get a driver’s license, a Social Security Number, or a Certification of Immigration Status letter, just let us know. If you need a verification of enrollment letter Records and Registration will generate an official school letter for you.
- OPT (Optional Practical Training) Application - Complete this application, then make an appointment with International Education. Please bring all required documents to apply for OPT to your appointment.
- OPT (Optional Practical Training) Employer Update - Tell us when you change jos or add a new job while on OPT. You can use this if you have technical difficulties with the SEVP OPT portal.

Program Extension - Please be sure to extend your program if you need to before your form I-20 expires. There are two forms for this process. Once both forms have been submitted to our office and approved, we can help with extending your form I-20 end date.
- Student I-20 Extension Request - this is for international students to request a program extension
- Academic Advisor I-20 Extension Recommendation - this is the form for academic advisors to recommend a program extension for their student
- Reduced Course Load (RCL) - Use this form to get approved for under enrollment (enrolled in less than 12 credits) for graduation quarter or for any medical conditions. You will need a doctor's note for any medical conditions to complete your approval process.
- Transfer-out Form - Transferring out to a new school? Please request to have your SEVIS record transferred. *You will need to attached a copy of your acceptance letter from the new school.

Vacation Quarter Request - You have been working really hard and are ready for a break and vacation quarter sounds like a great idea? Fill out the form for us!
- Eligibility: you must be a full-time student for three consecutive quarters to be eligible for a vacation quarter.
- You cannot take a vacation quarter in your final quarter or if you will not be returning to EvCC. If you need to enroll in fewer than 12 credits during your final quarter, you can apply for a Reduced Course Load.
- You may take below full-time course load during your vacation quarter, or take no classes at all.
